Pre-Submission Checklist

Before you submit your online consultation request, please confirm the following:

  1.  I’ve clearly described my main concern. I’ve stated what my main symptom or question is, when it started, and how it has changed over time.
  2. 🕒 I’ve included key details about timing and severity. I’ve mentioned how long I’ve had the issue, whether it’s getting better or worse, and how it affects my daily life.
  3. 💊 I’ve listed any medicines or treatments I’m using. I’ve included prescription drugs, over-the-counter remedies, supplements, and any self-care measures I’ve already tried.
  4. ⚠️ I’ve checked if my symptoms might need urgent help. I’ve ruled out red flag symptoms (e.g., chest pain, shortness of breath, severe bleeding, sudden weakness, confusion, new rash with fever).
If present, I’ll seek urgent or emergency care instead.
  5. 📸 I’ve attached clear photos if relevant.(e.g., skin rashes, swelling, wounds) — making sure the image is well lit and in focus.
  6. 🧾 I’ve added relevant background information. I’ve mentioned relevant medical conditions, allergies, or recent tests that might help the GP understand the context.
  7. 🧠 I’ve considered trusted self-care advice first. I’ve checked NHS.uk or my GP practice’s website for self-care and pharmacy advice and tried any recommended steps where safe.
  8. 🎯 I’ve made clear what I’m hoping for. I’ve stated whether I’m asking for advice, a prescription renewal, a test, a fit note, or an appointment.
  9. 🔁 I’ve avoided repeating a recent query. If I’ve already submitted this issue recently, I’ve checked any previous messages or advice before resubmitting.
  10. 📬 I’ve provided up-to-date contact details and availability. I’ve made sure my phone number and email are correct and that I’m reachable if the GP needs to contact me.

How does Patient Triage work?

It's easy for patients and practices

Patient Triage is managed by a technology company called accuRx who provide communications software to healthcare providers across the NHS. It can be used for booking an appointment or for any other query.

Patient Triage is recognised by the NHS and has passed the NHS Digital Capabilities assessment and allows registered patients to submit either a medical query or an administrative request to their GP practice.

It is a simple online form which takes a few moments to fill out.
