Your NHS Health Check results

Your NHS Health Check looks at your risk of developing conditions such as heart disease, stroke, kidney disease and diabetes.

Your results may show some changes you can make to improve your health.

The healthcare professional completing your Health Check will let you know which of the sections below are relevant to you.

Weight and healthy living

If your Health Check shows that losing some weight could benefit your health, there are free NHS and local services that can help.

Beezee Adults – Bristol

Beezee Adults supports Bristol residents to make realistic, sustainable changes to their eating, activity and weight.

Support includes online programmes and the Beezee Academy, which allows you to work through information at your own pace.

The Academy is available to Bristol residents aged over 18 with a BMI over 25, with a lower threshold of 23 for people from some ethnic minority backgrounds.

There is also Gutless, a free online programme specifically for men.

NHS Digital Weight Management Programme

The NHS Digital Weight Management Programme supports adults living with obesity who also have a diagnosis of diabetes, hypertension or both, to manage their weight and improve their health.

It is a 12-week online behavioural and lifestyle programme that people can access via a smartphone or computer with internet access

Smoking

Stopping smoking is one of the most important changes you can make for your health. You do not have to do it alone.

Smoke Free Bristol

Smoke Free Bristol provides free stop-smoking support for Bristol residents aged 12 and over.

Support includes:

  • One-to-one help with a stop-smoking practitioner
  • Nicotine replacement therapy
  • Stop-smoking medication
  • Vapes

How to get support

You can self-refer by:

  • Texting QUIT to 66777
  • Calling 0800 772 3437
  • Using the Smoke Free Bristol online service

There is also free NHS digital support available to help you stop smoking.

Alcohol

Your Health Check includes questions about alcohol.

If the result suggests that alcohol could be affecting your health, we can provide advice on reducing your intake and recommend further support where appropriate.

Horizons Bristol

Horizons Bristol provides support for adults in Bristol who would like help with alcohol or drug use.

You can self-refer directly through Horizons Bristol.

Online support

Online support is also available through Breaking Free for people in Bristol and South Gloucestershire.

If your Health Check suggests that you may need further assessment, we may recommend speaking to one of our clinicians.

Raised blood sugar / risk of diabetes

Your blood test may show that your blood sugar is higher than normal.

This does not necessarily mean that you have diabetes, but it can mean that you're at increased risk of developing type 2 diabetes.

What happens next?

If your result is raised, the practice will advise you about the appropriate next steps. These may include:

  • Lifestyle changes
  • Further monitoring
  • Referral to appropriate diabetes support

Maintaining a healthy weight, eating well and being physically active can all help to reduce your future risk.

If your results suggest diabetes rather than increased risk alone, a clinician will arrange the appropriate follow-up.

BNSSG also has diabetes self-management and specialist community services available for people who need additional support.
