Blood Pressure Review

If we've asked you to check and submit your blood pressure readings, please don’t delay. You can return your readings to us via the SMS questionnaire we sent you, or if the link has expired, please submit the form below.

Please take your reading twice a day (morning and evening) for the time perioid specified in the text we sent you (either 4 days or 7 days).

Why checking and managing your blood pressure matters

High blood pressure (hypertension) often has no obvious symptoms, but if left uncontrolled, it can cause serious and lasting damage to your heart, brain, kidneys, and eyes. It’s a major risk factor for heart attacks, strokes, kidney disease, and other life-threatening conditions.

That’s why it’s so important to regularly check your blood pressure—not just for those already diagnosed, but also to identify people who may be living with high blood pressure without knowing it. Many people feel fine, even when their blood pressure is dangerously high.

Early identification and proper management are key. If we detect high readings early, we can work together—through lifestyle changes or medication where needed—to bring your blood pressure under control. This helps reduce your risk of serious complications and supports you in leading a longer, healthier life.

Your readings give us vital information to help protect your health.
